This form describes the code of conduct for Beautiful Feet International Inc. mission trips. Participants are expected to follow all rules cited in this document. Each mission trip participant must provide a signed copy of this form.

Gives us consent to take your child with us on one of our mission trips.

Authorizes Beautiful Feet to use or post photos, videos or your testimony, on the website, social media or elsewhere.

Application to join a Beautiful Feet International Inc. mission trip. A copy of this application must be turned in before EVERY mission trip (even if you are a returning member).

This form gives doctors the right to treat you at the mission trip location.
